Fever of 100 or higher?

Have acute respiratory symptoms? e.g., cough, difficulty breathing, etc.

Loss of taste or smell?

In the last 14 days:

Have you had a close contact with a suspected or confirmed COVID-19 patient?

Have you traveled from an affected geographic area?

Have you been on a cruise ship, airplane, bus, or have you attended a group gathering?

Are you age 60 or over?

Do you live in a communal setting such as a long term care facility, group home, or shelter?

Do you have a chronic medical condition or are you considered immunocompromised? Examples include, but aren't limited to diabetes, heart disease, chronic kidney disease, chronic lung disease, or taking immunosuppressive medication.
